VA Disability DBQ Examinations
Independent Medical Evaluations for Veterans
Comprehensive Disability Benefits Questionnaire (DBQ) evaluations designed to help veterans document their medical conditions, symptoms, and functional limitations.
At Bridgeport Wellness Center, we understand how important accurate medical documentation is when pursuing VA disability benefits. Our independent DBQ examinations provide veterans with thorough, individualized medical assessments that can be submitted as supporting evidence for VA disability claims.
Whether you are filing an initial disability claim, requesting an increased rating, or documenting a secondary medical condition, we are here to help ensure your medical findings are accurately and professionally documented.
What Is a VA Disability Benefits Questionnaire (DBQ)?
A Disability Benefits Questionnaire (DBQ) is a standardized medical form developed by the Department of Veterans Affairs to document the severity and clinical characteristics of a medical condition.
DBQs help the VA evaluate medical evidence when determining disability ratings.
Our independent examinations focus on:
-
Comprehensive Medical Assessments: Detailed evaluations of your current medical conditions and symptoms.
-
Functional Limitations: Documentation of how your condition affects mobility, employment, daily activities, and overall quality of life.
-
VA-Standardized Documentation: Completion of applicable VA Disability Benefits Questionnaires using clinical findings.
-
Medical Record Review: Review of relevant medical history, diagnostic testing, and previous treatment.
-
Objective Clinical Findings: Physical examination findings, range-of-motion measurements, and other appropriate assessments.
-
Personalized Evaluations: Individualized appointments that allow time to thoroughly discuss your symptoms and medical concerns.
Who Can Benefit From a Private DBQ Examination?
Our independent DBQ examinations may be beneficial for veterans who are:

DBQ Examinations vs. Nexus Letters
Although both services can provide valuable medical evidence, they serve different purposes.
DBQ Examination: Documents the diagnosis, symptoms, severity, and functional impact of a medical condition. This information may help the VA determine an appropriate disability rating.
Nexus Letter: Provides an independent medical opinion regarding whether a medical condition is connected to military service or to an existing service-connected disability.
Some veterans may benefit from both services depending on their individual claim circumstances.

Important Information
Bridgeport Wellness Center is an independent medical practice and is not affiliated with, contracted by, or endorsed by the U.S. Department of Veterans Affairs.
Our private DBQ examinations are not official VA Compensation & Pension (C&P) examinations. The VA determines the evidentiary value of submitted DBQs and may require additional examinations or documentation.
Submission of a private DBQ does not guarantee service connection, an increased disability rating, or approval of VA benefits. Certain DBQs are not available for private-provider completion, and some evaluations require specialized examiners or testing.
All medical findings and documentation are based on independent clinical judgment.
