VA Nexus Letters & Independent Medical Opinions

Helping Veterans Establish the Medical Connection Between Their Military Service and Current Health Conditions

Professional, evidence-based medical opinions to support VA disability claims.

At Bridgeport Wellness Center, we understand that obtaining VA disability benefits often requires more than simply having a medical diagnosis. Veterans may also need medical evidence demonstrating how their current health conditions are connected to military service or an existing service-connected disability.

Our independent VA Nexus Letter services provide comprehensive medical record reviews and professionally written medical opinions to help veterans present supporting medical evidence for their disability claims.

Whether you are filing an initial claim, pursuing secondary service connection, or appealing a previous denial, our goal is to provide thorough, objective medical documentation based on your individual medical history and available clinical evidence.

What Is a VA Nexus Letter?

A VA Nexus Letter is a written medical opinion from a qualified healthcare professional addressing the relationship between a veteran's medical condition and their military service or an existing service-connected disability.

A well-supported Nexus Letter may provide important medical evidence when the VA evaluates whether a condition qualifies for service connection.

Our Nexus Letters may include:

  • Comprehensive Medical Record Review: Evaluation of relevant military service records, medical history, diagnostic testing, and treatment documentation.

  • Independent Medical Opinion: An objective assessment of whether the available evidence supports a relationship between a medical condition and military service.

  • Evidence-Based Medical Rationale: Clinical explanations supported by relevant medical research, established medical principles, and individual findings.

  • Direct Service Connection: Assessment of whether a medical condition may be related to an injury, illness, exposure, or event during military service.

  • Secondary Service Connection: Evaluation of whether an existing service-connected condition caused or aggravated another medical condition.

  • VA-Recognized Medical Terminology: Use of appropriate medical probability language, including "at least as likely as not," when supported by the evidence.

  • Individualized Documentation: Medical opinions tailored to the veteran's specific diagnoses, history, and circumstances.

Types of VA Nexus Letters We Provide

Direct Service Connection

A direct service connection involves a medical condition that began during military service or developed as a result of an injury, illness, exposure, or event related to service.

Examples may include:

  • Chronic back pain following an in-service injury

  • Joint conditions associated with documented military injuries

  • Migraines associated with a service-related head injury

  • Respiratory conditions potentially associated with qualifying exposures

Our evaluations consider relevant service records, medical documentation, and the available evidence of a relationship between military service and the current condition.

Secondary Service Connection

A secondary service connection may apply when an existing service-connected disability causes or aggravates another medical condition.

Many veterans develop additional medical conditions that may be medically related to disabilities for which they already receive VA benefits.

Examples of relationships that may warrant individualized medical evaluation include:

  • Sleep apnea and certain service-connected medical conditions

  • Migraines associated with cervical spine disorders

  • Peripheral neuropathy associated with diabetes

  • Radiculopathy associated with lumbar spine conditions

  • Mental health conditions associated with chronic pain

  • Gastrointestinal symptoms potentially associated with medications used to treat service-connected conditions

  • Joint disorders potentially associated with altered gait or biomechanics

These relationships are not automatically presumed. Each case requires an individualized review of medical evidence and alternative explanations.

Aggravation of a Preexisting Condition

In some circumstances, military service may have worsened a medical condition that existed before enlistment.

A Nexus Letter may address whether the available medical evidence supports aggravation of the preexisting condition during military service, including whether the worsening was beyond the condition's natural progression.

Secondary Aggravation

A service-connected disability does not always have to directly cause another condition to be medically relevant.

In some cases, an existing service-connected condition may worsen a separate medical disorder.

Our medical opinions can assess whether the clinical evidence supports this relationship and, when possible, describe the degree of worsening.

Medical Conditions We Review

Our independent medical opinion services may include evaluations involving:

  • Chronic back and neck disorders

  • Degenerative disc disease and spinal conditions

  • Arthritis and joint disorders

  • Migraines and chronic headaches

  • Sleep apnea and sleep-related disorders

  • Hypertension and cardiovascular conditions

  • Diabetes and metabolic disorders

  • Peripheral neuropathy and radiculopathy

  • Chronic pain conditions

  • Certain mental health conditions

  • Gastrointestinal disorders

  • Other conditions within our provider's clinical scope of practice

Acceptance of a case depends on the medical condition, available records, clinical complexity, and whether the requested opinion falls within the provider's qualifications.

What Makes a Strong VA Nexus Letter?

A Nexus Letter is more than a statement that two medical conditions might be related.

A credible medical opinion should provide a clear explanation of how the available evidence supports the medical conclusion.

At Bridgeport Wellness Center, we emphasize:

1. Thorough Medical Evidence Review

We review relevant medical records, service treatment records, prior diagnostic testing, and clinical documentation.

2. Individualized Medical Analysis

Every veteran's medical history is different. Our opinions are developed around the specific facts of each case rather than generic templates.

3. Supporting Medical Literature

When appropriate, we incorporate relevant peer-reviewed research and established medical principles to support the clinical rationale.

4. Clear Medical Probability Statements

We use medically appropriate probability language when the available evidence supports a conclusion.

5. Objective Clinical Judgment

Our opinions are based on the medical evidence, including consideration of alternative causes and relevant risk factors.

6. Professional Medical Documentation

Each completed opinion is written to clearly communicate the medical findings, reasoning, and conclusions.

Who May Benefit From a Nexus Letter?

An independent medical opinion may be useful for veterans who are:

  • Filing an initial VA disability claim

  • Seeking to establish secondary service connection

  • Submitting new and relevant evidence with a supplemental claim

  • Addressing a previous denial involving insufficient medical nexus evidence

  • Seeking clarification of conflicting medical opinions

  • Documenting a potential relationship between an existing service-connected disability and another diagnosis

  • Pursuing a claim involving aggravation of a medical condition

Nexus Letter vs. DBQ Examination

Both services may provide useful medical evidence, but they serve different purposes.

Nexus Letter

Addresses whether a medical condition is medically connected to military service or another service-connected disability.

Disability Benefits Questionnaire (DBQ)

Documents the current diagnosis, symptoms, severity, and functional limitations of a medical condition.

Depending on the circumstances, a veteran may benefit from a Nexus Letter, a DBQ, or both.

Frequently Asked Questions

Do I need a Nexus Letter for my VA disability claim?

Not every VA disability claim requires a private Nexus Letter. However, an independent medical opinion may be helpful when the relationship between a medical condition and military service is unclear, disputed, or insufficiently documented.

Can a Nurse Practitioner write a VA Nexus Letter?

Yes. A qualified Nurse Practitioner may provide an independent medical opinion within their scope of practice and clinical expertise. The VA evaluates medical opinions based on factors such as the provider's qualifications, review of relevant evidence, and the quality of the medical reasoning.

Certain medical questions may require additional specialist expertise.

Can I obtain a Nexus Letter without an in-person appointment?

Some medical opinions can be developed through a comprehensive records review and remote consultation. Other cases may require an in-person examination, additional diagnostic testing, or specialist evaluation.

Can a Nexus Letter help with a previously denied claim?

A well-supported medical opinion may provide new and relevant evidence for a supplemental claim, particularly when a previous denial identified insufficient evidence of a medical connection.

A Nexus Letter does not guarantee that the VA will reopen, grant, or approve a claim.

Does a Nexus Letter guarantee a disability rating?

No. The VA independently determines service connection, disability ratings, and eligibility for benefits. A Nexus Letter provides medical evidence for consideration but does not guarantee an outcome.

Do I need a VA referral?

No. Veterans can request an independent private medical opinion directly through our practice.
